This form is used to submit your Quarterly and/or Closeout Financial Report.
If you wish to use this form to submit your Closeout Financial Report, please indicate this by checking the box in the Closeout Reporting Section.
If you wish to use this form to submit a final Quarterly Financial Report that will ALSO serve as your Closeout Financial Report, please enter the appropriate quarter in the Quarter Being Submitted field and then ALSO select Yes in response to "Is this a Closeout Financial Report for this Award?"
If there were expenses beyond the final quarter to include in Closeout reporting, these expenses should be listed in the first column (Grantee Expenditures). The Prior Expenditures column would include all previously reported quarters.
