 Semester Special is a use or lose special. Missed classes will not be carried over to the next semester or refunded. You will have the flexibility to make up your personally missed classes to include rain days during that semester. Those signing up for Full-time will not have this ability because the per class rate has already been greatly discounted.

Please fill out to reserve your spot in our class being held at Lee LeClear Tennis Center. Limited spots are available.We will continue our commitment to providing quality instruction. We will limit the number of players per day and will not have open enrollment. All players must be approved before being admitted into our program. Payment must be received before your spot is secure.

Child Information

The McAdoo Academy is not an open program. We have a limited number of players allowed per month to ensure a quality training environment. Once you secure your days, those days are removed from my inventory of open space. I cannot limit the number of players and also extend credits for missed classes. Please note there will be no make-ups or carry-over days to the following sessions for any reason except for classes we could not provide. You are paying for your sessions in advance, which secures your place for that month. There will be no credits given for missed sessions due to school, vacations, etc... This will be strictly adhered to, so please contact me with any questions before signing up for your session.

RAINOUTS

In the event of rain, we will attempt to hold a zoom class. If the Zoom class is not able to be done, we will schedule weekend make-up classes when possible. If neither of the options above are possible, credits will be given.

MAKE-UPS

There will be no makeup credits given for any reason. You are welcome to attend all weekend make-ups and zoom sessions when they are held. 

FINANCIAL RESPONSIBILITY and CANCELLATION POLICY

Please be aware that the above registration is a financial commitment to our program. Please note each month has a date to cancel by or make changes to your registration. If you sign up for a month and no show or attempt to drop the month after cancellation date listed you are still financially responsible for that registration. Acknowlege and Agreement to above billing, cancellation and make-up policy

I will do everything in my power to honor this commitment. If my schedule changes and I cannot keep this registration, I will immediately contact Bernard McAdoo and inform him of the change. I also understand that no refunds or credits for missed classes (except for weather-related cancelations), will be given or carried over to other weeks. I fully understand that I am financially responsible for the above registration.

 

Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by McAdoo Academy of Tennis during the selected program. In exchange for the acceptance of said child’s candidacy by  McAdoo Academy of Tennis., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless McAdoo Academy of Tennis . and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected sessions. In case of injury to said child, I hereby waive all claims against McAdoo Academy of Tennis . including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including Tennis. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment and x-ray examination.  In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the  McAdoo Acadmey of Tennis and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
