I, the undersigned, acknowledge and agree to the following:
I am 18 years of age or older and have provided valid government-issued ID.
I understand the risks involved with getting a tattoo, including infection, allergic reactions, scarring, and dissatisfaction with the outcome.
I confirm that I am not under the influence of drugs or alcohol.
I have informed the artist of any medical conditions or allergies.
I understand that tattoo healing is my responsibility and have been given aftercare instructions.
I release the artist and studio from all liability related to the procedure.
I consent to photos being taken of my tattoo for portfolio or promotional use
