Reappointment/Merit section
of the letter should contain a report of the faculty opinion and vote and an evaluation of the candidate's performance in teaching, research or creative work, and service.
Midcareer Appraisal section
of the letter should be clearly labeled as "Positive," "Provisional Positive," "Guarded," or"Negative." This section should assess the candidate's prospects for promotion; contain a report of the faculty opinion and vote and evaluation of the candidate's performance in the following areas: teaching, research, or creative work, professional competence and activity, and University and public service. (The letter and vote should have notation that a copy has been provided to the candidate.)
