2025-2026 Birthday Shout-Out Request

Birthday shout-outs are a fun way to celebrate your child while they’re at school! Please complete this form and submit cash only in a labeled envelope or bag with your child’s full name, grade, and amount enclosed. Payments must be delivered to Mrs. Hoyt in the front office by Friday at 3:00 p.m. prior to the upcoming week. If there is a school break, payments are due the Friday before the break, as the video is finalized by 3:15 p.m. that day.

Videos cannot be changed once uploaded, and late submissions or payments will not be honored. If received late, your child’s shout-out will be moved to the following week. Carpool birthday signs are limited and will be assigned in the order payments are received when multiple students share the same birthday.
