EcoRecord Order Form

INSTRUCTIONS:

This order form is organized into 3 sections: Step 1 - records (just the records, excluding print items); Step 2 - print items (sleeves, jackets, inserts, etc); Step 3 - misc and assembly items (shrinkwrap, polybags, bar codes, etc).
Check the box to the left of the item that you want to add to your order, then use the drop-down menus to select the qty and any options.
Make sure that you select the same qty for each item if you want them to match up. (i.e, if you order 300 records, make sure you order 300 jackets if you want all 300 copies to be jacketed).
After making your selections, you will check out, and then complete some questions that specifically pertain to your record (artist and title, matrix number, shipping information, etc).
After submitting this form, we will check your order and contact you within 72 business hours for confirmation.
The turn around time depends on the ordered items, and will be confirmed when we contact you.
Taxes and shipping costs will be added to the final invoice when your order is ready to ship.

GGR Hosted Download CardsEach record will have a coupon with a thumbnail of your album art, and a unique code that is redeemable one time for the user to download a digital copy of your music. You can supply both mp3 and high res audio files, as well as a video file. Your record will have its own unique landing page, with the URL printed on the coupon for your buyers to redeem their codes.

STEP 3: MISC & ASSEMBLY ELEMENTS (Shrinkwrap, Stickering, UPC's, etc)

Bar Code RegistrationA unique bar code to identify your record. After payment is received, we will provide a graphic file for you to place onto your artwork, if you want the bar code to be printed. You can order stickers separately if you would prefer to print the bar code on them.

Shrinkwrap ($0.35 per unit)Sealed shrinkwrap around your album. Keep in mind - most purchasers ultimately throw shrinkwrap in the trash. Consider polybags to reduce plastic waste. (Gotta Groove uses ExlfilmPlus GPS shrinkwrap, which is made in part with pre-consumer recycled content, and can be recycled at specific locations in the USA and Canada through the How2Recycle network.

Scribe Number / Matrix Inscription

This is the number/letter combination that will be scribed into the deadwax area of the record next to the label. There are no real requirements/limitations, other than it needs to be unique. If you cannot think of anything, just type -please assign-; and we will set the matrix number for you. VERY IMPORTANT: If you are supplying lacquers, it is extremely important that what you type here matches precisely what your lacquer engineer is scribing into the lacquers. Also, if there is any additional text you want scribed into the leadout, this is where to put this information (just keep in mind the area is small. (We do our best to accommodate these requests, and we do not charge extra for matrix messages. But, please understand that there can sometimes be slight differences between the text you type here, and what is actually scribed).

TRACK ORDER / SIDE SPLIT / TRACK INFORMATION

If you are sending us individual tracks, please tell us their order, and which side they should be placed. If you are sending a single file for each side of the record, please send us a TIMELOG for each side of the record. (The timelog should show start/duration/end of each track, and side B should start over at ZERO).

NOTE: THE DEFAULT SHIPPING METHOD FOR TEST PRESSINGS IS UPS GROUND (For Domestic Addresses) OR USPS FIRST CLASS INTERNATIONAL (For International Addresses). If you want a different shipping method, such as overnight air, please tell us in the box above now. Thank you!

FINAL SHIPPING ORDER ADDRESS(ES) (Required)*

If ordering 300 or more records, please include answers to the following questions: 1. Commercial or Residential Location? 2. Delivery Hours (must be a timeframe - can't be "anytime"). 3. Is a Call Ahead Appointment Required (someone must be present to take delivery if sent via a freight service). 4. Does the location have a trailer-height loading dock?.

INCLUDE CARRIER PROVIDED INSURANCE?

Carrier provided insurance covers visible damage to shipping cartons. In the case of filing a damage claim, you must keep the shipping cartons until the claim is complete, or the claim will be denied by the respective carriers. When a claim is honored, the carriers insure only the manufactured value of the goods - not the retail or wholesale value. All orders ship F.O.B. Gotta Groove Records, Cleveland, Ohio -- this means that the risk of damage/loss shifts to the customer (you) once the packages are picked up from the GGR pressing plant. Cost for insurance varies depending upon the quantity of the order, but is rarely more than $30-50 for orders up to 2,000 records.

TRIPLE BOX ORDER?

All orders are double boxed by default. You can opt for triple boxing for $5 per carton. Orders shipped on a pallet do not typically need to be triple boxed. 12 inch record orders including jackets and not shipped on a pallet will REQUIRE triple boxing @ $5 per carton - this charge will be added to your final invoice if your order is a 12 inch record order including jackets, and not shipping on a pallet.

INSTRUCTIONS FOR EXTRA PARTS / OVERRUN PRINT (Required)*

UNCLAIMED PARTS LEFT AT GGR WILL BE DISCARDED 90 DAYS AFTER SHIPMENT *WITHOUT NOTICE* UNLESS PRIOR WRITTEN ALTERNATIVE ARRANGEMENTS HAVE BEEN MADE. GGR will store mothers for at least three years, BUT NOTE MOTHERS ARE NOT GUARANTEED FOR RE-USE. Otherwise, GGR does not store any customer parts, and parts left at GGR premises will be discarded within 30 days of order shipment. Please note -- on smaller quantity orders (under 500 records), there are often leftover print parts far in excess of the order quantity. These will be shipped back to you by default, so if you do not want them, please be sure to speak with a GGR representative about them before your order ships.

SALES TAX ACKNOWLEDGEMENT (Required)*

If you are domiciled in Ohio and/or if you are picking up your order from GGR, you WILL be charged sales tax unless you provide a signed Ohio Blanket Certificate of Exemption. This form is available on the Gotta Groove website on the Order Form page. Please do not forget to provide this form when you place your order, or sales tax will automatically be added if you are domiciled in Ohio or if you are picking up your order.
