Definitions and Instructions:
Equipment means tangible personal property (including information technology systems) having a useful life of more than one year and a per-unit acquisition cost which equals or exceeds the lesser of the capitalization level established by the non-Federal entity for financial statement purposes, or $10,000 [2 CFR 200.1 Definitions, Equipment {November 12, 2020 per 44 Ill. Adm. Code 7000.40 (c) and (d)}].
Once the present value is reported at less than $10,000.00, the item can cease to be reported.
Use and condition should portray active use (yes/no) and indicate the condition of the equipment using "new," "good," "fair," "poor," or "scrap."
Source(s) of funds may include grant funds, internal resources, other awards (please name them.)
Location would be brief description like Building/Floor/Room# or if ownership/title of the equipment has been transferred, please name the current entity possessing the equipment. Another location could be "disposed."
Method of disposal could include sold (include price), donated, scrapped, recycled, landfilled or incinerated.
