The Arc in Hawaii and all information providers (including former employers, educational institutions, and personal references) are released from any liability related to or arising out of these inquiries.

4. I understand that if an employment offer is made, I will be required to submit to and pass a drug test (at The Arc in Hawaii’s expense) prior to moving forward with pre-employment. The Arc in Hawaii will handle these results confidentially, disclosing them only to those with a need to know or as legally required.

5. I understand that this application will be considered active for three months. To be considered for employment after this time, a new application must be submitted.

6. I understand that if hired, all the terms and conditions outlined in this certification will become part of my employment relationship with The Arc in Hawaii.
