同意條款 Terms of Agreement:

本人了解並同意佛光山寺及派下各單位，得將本人所填寫包括先前已提供及未來提供的個人資料蒐集、處理、使用在功德徵信、法會、活動及未來各項活動通知、帳務處理與功德主提報等作業。且本人提供之本人以外其他個人資料，本人已以適當方式使該人等知悉及同意其個人資料會提供予佛光山寺派下相關單位，進行蒐集、處理或利用。

I understand and agree to Fo Guang Shan Monastery (FGS) and all affiliated branches and departments, may collect, process, and use the personal data that I have provided, including previously provided and provided in the future, for merit registration, ceremonies, activities and such as notification of activities, accounting processing and reporting of merits. In addition, the personal information of others provided by me, have been informed and agreed that the personal information will be provided to the relevant units of Fo Guang Shan Monastery and all affiliated branches and departments for collection, processing or use.

責任免除聲明 Waiver of Liability:

本人在此同意並確認，永遠免除多倫多佛光山（又稱“I.B.P.S.Toronto”）、其所屬之法師、講師、工作人員及義工對本人不承擔任何直接或間接責任。本人放棄因參加課程所引起或以任何方式相關之事件，對本人造成的任何人身傷害、個人財產之損失、或由此而生之索賠、損害、要求、訴訟及法律行動。

I hereby agree to release and forever discharge, Fo Guang Shan Temple of Toronto (also known as “I.B.P.S. of Toronto,”) its respective Buddhist monastics, instructors, staff and volunteers from all liability, whether direct or indirect, and hereby waive all claims, demands expenses, actions and cause of action which may arise from any injury to myself, and any loss or damage to personal property arising from, or in any way resulting from, my participation in the event, activity or class(es).
