Liability Waiver and Release
I, the undersigned participant or parent/legal guardian of the above-named participant, acknowledge that participation in Vacation Bible School (VBS) activities at First Baptist Church of Lyman involves certain inherent risks, including but not limited to physical activity, games, crafts, and group interactions.

I voluntarily assume all risks associated with participation in these activities.

I hereby release, waive, and discharge First Baptist Church of Lyman, its staff, volunteers, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damage that may occur during participation in VBS, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Treatment Authorization
In the event of an emergency, I authorize First Baptist Church of Lyman staff and volunteers to obtain necessary medical treatment for the participant. I understand that every effort will be made to contact me or the emergency contact listed above.
Photo and Media Release
I grant permission for First Baptist Church of Lyman to use photographs or video recordings of the participant taken during VBS for promotional or ministry purposes, including social media, print materials, and church publications.
