Adoption Form- DOGS 2026

Please fill out the adoption form below. We ask that you be honest—it's crucial that we place these animals in the best homes where both they and their families will thrive. Our goal is to find forever homes for our animals, where they will never have to be surrendered again.

Please note: Adoption fees are non-refundable.

Veterinary Care Provided:
Dogs: Spayed/neutered, microchipped, dewormed, treated for fleas and ticks, and vaccinated.
Puppies: Spayed/neutered, microchipped, dewormed, treated for fleas and ticks, and receive their first set of vaccinations.

Once the adoption is complete, all veterinary records will be sent to your adoption chat. Microchip information will also be sent, and it is the adopter’s responsibility to update the ownership details.

Riverview Rescues does its best to assess the health and behavior of every animal in our care. However, we cannot be held responsible for any health or behavioral issues that arise after adoption. If you adopt an animal with known health or behavioral concerns, it becomes your responsibility once the adoption is finalized.

We ask that you remain in contact with the rescue for updates. Riverview Rescues is not liable for any damage to personal property, injuries to individuals, or legal issues resulting from bites or attacks while the animal is in your care. All dogs must be leashed until they are trained with a reliable recall and must be supervised at all times.

Please complete the form in full and read the description of the animal you are applying for carefully.
If the form is incomplete or if your answers do not align with the animal’s needs, your application will be deleted without notification.

Adoption Fees:

Dogs: $700.00
Puppies (12 months and under): $800.00
Foster-to-Adopt (FTA) Fee: $100.00 (non-refundable; applicable only in certain cases and credited toward the adoption fee if finalized)

We do not adopt littermates to the same home.
We will not place a puppy in a home that already has another puppy under 1 year old.

BEFORE PROCEEDING: By continuing with this form, the applicant acknowledges and agrees that early sterilization (for puppies) and sterilization of adult dogs is mandatory and cannot be postponed, rescheduled, or altered in any way. Adoption contracts are not considered valid or complete until sterilization has occurred. Please ensure you have done your research and are comfortable with this requirement before moving forward with the application.*
