This form will help me understand you as a client and better create a plan that is tailored to you in order for you to meet your goals. This form is only for those who are looking for an intensive elite level body transformation. You must be willing to dedicate significant time to training and to tracking your nutrition and being very detailed in your execution of your plan. If you are filling this form out for an intense transformation, a consultation of about 30 to 45 minutes will follow the completion of this form.
