I hereby certify that I am the parent or guardian of the children named above, and I do hereby give my consent without reservation to Resurrection Greek Orthodox Church to use the likeness of my children in photographs, video recordings or electronic images in any and all of its publications, social media sites (Instagram, Facebook, etc.), digital signage on the church grounds, and the Resurrection GOC website, including any photos taken during Resurrection's Youth Ministries & Sunday School program. If said photos are posted, the last names of youth under the age of 18 will not be included.
