Membership Fees:
The monthly membership fee is $99.00. Fees are subject to change with a 30-day notice provided to the member.
Payment Schedule:
Membership fees will be billed on a recurring basis every 30 days, starting from the effective date of membership. Payments will be automatically charged to the member’s preferred payment method on file.
Terms and Conditions
Cancellation Policy
Members may cancel their membership at any time by providing a 30-day written notice to support@elysianintegrativehealth.com. Cancellation requests must be submitted in writing or via email to the clinic. Membership will remain active during the 30-day cancellation period, and all fees due during that time will be charged.
Non-Transferable
Memberships are non-transferable. Only the individual named on the membership may use the services and discounts provided.
Grounds for Termination
The clinic reserves the right to terminate a membership without notice for reasons including, but not limited to, breach of these terms and conditions, abusive behavior toward staff, failure to comply with clinic policies, or failure to maintain payment. Any unused services or benefits will be forfeited upon termination by the clinic.
Refund Policy
There will be no refunds issued for membership fees or unused services. Banked fees can be applied toward future treatments, but unused services will not be refunded.
Member Responsibilities
Members are responsible for maintaining up-to-date contact and payment information with the clinic.
Members agree to show up for scheduled appointments and notify the clinic 24 hours in advance if they cannot attend.
Members are responsible for using their benefits within each membership period. Services not used within a month can be banked for future use but cannot be transferred or refunded.

We take your privacy seriously. All personal information provided by the member will be kept confidential and used solely for the purposes of membership management and providing services.
