Responsibility Declaration:
I, the undersigned, hereby declare that:
1. I am the legal owner of the above-identified animal and I take full responsibility for the animal during its stay at Le Petz – Hotel das Mascotes.
2. I confirm that the animal is properly vaccinated, dewormed (internally and externally), and shows no symptoms of contagious diseases at the time of check-in.
3. I authorize the hotel staff to seek veterinary services in case of emergency if I cannot be contacted in time. I agree to fully reimburse any veterinary expenses incurred.
4. I declare that the animal does not show aggressive behavior toward people or other animals, unless explicitly stated below:
5. I commit to informing the hotel in advance about any special dietary, medical, or behavioral needs of the animal.
6. I accept that the hotel cannot be held liable for any illnesses the animal may develop, provided all hygiene and safety standards are followed.
7. I understand that failure to collect the animal on the agreed date may result in additional charges, and after 30 days without contact, the case may be considered abandonment and referred to the appropriate authorities.

I authorize my animal to participate in socialization activities with other animals of the same species, under the supervision of the hotel staff. I understand that all precautions will be taken to ensure safety, but I acknowledge that there is always an inherent risk in animal interactions.
