CSP FC INC. – Registration Agreement & Waiver I authorize CSP FC INC. to collect and use personal information about me and/or my child/ward for the purpose of receiving communications from CSP FC INC.. I understand and agree that CSP FC INC. does not sell or disclose any personal information to third parties. By signing this agreement, I acknowledge that I have read and agree to the Policies, Terms, and Conditions of CSP FC INC.. As the parent or legal guardian of the registered player, I affirm that the player is in good health and capable of participating in competitive soccer activities. I fully understand and acknowledge that participation in such activities involves inherent risks, including but not limited to: Risk of serious injury, including permanent disability and death.Severe social and economic losses. Risks arising from one’s own actions or inactions, the actions or inactions of others, the rules of play, the condition of the premises, or the equipment used. Other unforeseen risks not known or reasonably foreseeable at this time I voluntarily assume all such risks and accept full personal responsibility for any injury, permanent disability, death, or damage that may result from participation. I hereby release, waive, discharge, and covenant not to sue CSP FC INC., its teams, administrators, directors, agents, coaches, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, the owners and lessors of premises used to conduct events (collectively referred to as the “Releasees”) from any and all claims, demands, losses, or damages arising from or related to injury, including death or damage to property, whether caused by the negligence of the Releasees or otherwise. By signing below, I confirm that I have read this Registration Agreement & Waiver in its entirety, understand its terms, and agree to be legally bound by it. I acknowledge that I am the parent or legal guardian of the registered player and am executing this agreement voluntarily, even if I have not read the full document. I acknowledge and agree that CSP FC INC. may photograph, record, or otherwise capture the likeness, voice, or image of me and/or my child/ward during training sessions, games, tournaments, or other club-related activities. I grant CSP FC INC. the unrestricted right and permission to use, reproduce, publish, or distribute such media in any format (including but not limited to print, digital, and social media) for the purposes of promotion, marketing, publicity, and club communications. I understand that no financial compensation will be provided for the use of such media. I further waive any right to inspect or approve the finished product, including written or electronic copy, wherein my likeness or my child/ward’s likeness appears. If I wish to withdraw this consent at any time, I understand that I must provide written notice to CSP FC INC.
