    Please review the program objectives, potential risks, and agreements below. By signing, you acknowledge understanding and acceptance.
  • PROGRAM OBJECTIVES: I understand that this physical fitness program includes exercises to build cardiovascular endurance, muscular strength, flexibility, and improve body composition. All activities will be explained and demonstrated, and I can ask questions at any time.

    Potential Risks & Assumption of Risk: I understand that participating in physical exercise carries risks, including but not limited to heart, lung, or blood vessel abnormalities (e.g., heart attacks, blood pressure changes); musculoskeletal injuries (e.g., strains, tears, broken bones); heat prostration; shin splints; or other illnesses/injuries, including death. These risks may arise from equipment use, heavy calisthenics, or negligence. I voluntarily assume all such risks, including those from using facilities, equipment, or premises, and release Trainer from liability for any resulting damages or injuries.

    Release & Indemnification: I agree to release and hold harmless JJB Enterprises, Inc. DBA Coach Julia (Trainer), its affiliates, and the Facility (NCH Wellness) from all claims, damages, or losses arising from my participation, including those from negligence. I indemnify Trainer against any costs from such claims. I confirm I am in good physical condition with no impairments preventing exercise, and I have completed the PAR-Q honestly, seeking medical advice if needed.

    This section outlines my coaching philosophy and expectations for both of us to ensure your success.
  • Coaching Philosophy: My coaching philosophy revolves around empowering clients to become the best versions of themselves through a holistic approach combining physical exercise, nutrition, and self-care. I believe in building strength, resilience, and confidence from the inside out, with realistic goal-setting, consistency, and progress over perfection. My style is professional, friendly, supportive, and includes tough love when needed to push you beyond limits.

    What is Expected of the Client: Start sessions on time and preparedwith your best for that day. Be willing to adjust goals as we progress. Respond to challenging questions honestly. Hold yourself accountable for actions and inactions. Keep me inform about your physical and mental health as we go, so I can adapt as needed. 

    What is Expected of the Coach: Start sessions on time and with a plan. Listen to your needs and work within them. Educate as we go. Ensure your training is safe and maintain a safe training enviroment. Help overcome obstacles, clarify goals, and develop action items. Sessions will focus on progress toward your objectives.

  • Program Policies

    To ensure smooth operations and fairness for all clients, please review the following policies.
  • Lateness Policy: It is required that personal training sessions start on schedule. A session will be forfeited if you arrive 15 minutes late for a 60-minute session. If the client is late, the trainer has the right to leave the training facility. Unless prior arrangements have been made with Coach Julia, clients who arrive after their scheduled session has begun will only receive the time that is left. If Coach Julia is running late, the client shall be owed back the missed time at a time suitable to both the trainer and the client.

    Rescheduling and Cancellation Policy: Rescheduling or cancellation of appointments must occur at least 12 hours before your training session. Clients who do not reschedule/cancel with 12 hours’ notice will be charged for the session or lose it from their purchased package. Coach Julia understands that emergencies happen and may omit the charge at her discretion. Subsequent short-notice cancellations will be charged or lost. No-shows are not eligible for make-ups. Appointment changes prior to 12 hours before your schedule session can easily be changed/cancelled using the sceduling features at MyCoachJulia.com. If you need to make a change within 12 hours of your sceduled session, please call/text: (239) 207-1837 directly. 

    Expiration Policy: Packages and personal training sessions must be used within 120 days of the purchase date. After this time, personal training sessions and packages become invalid. There is no transferability for any personal training sessions or packages.

    Refund Policy: Packages and personal training sessions are not refundable. In cases of injuries or changes in medical condition leading to physical limitations, exceptions are at the trainer's discretion. Discuss your situation with Coach Julia!

  • All personal information is confidential and complies with applicable privacy laws. Your participation is voluntary, and you assume responsibility for consulting a doctor if needed.
