Booking and Admission Process – Tales of Tails Veterinary Drive. In partnership with Rooikoppies Animal Hospital. To participate in the upcoming veterinary drive, please begin by completing a mandatory e-booking via The Blue Collar Pet Shop Lenasia. Once your details have been submitted, kindly await a confirmation message from our administrative team. This message will indicate whether your booking has been approved. If your booking is successful, please visit The Blue Collar Pet Shop Lenasia (28 Ivy street) on the scheduled day of the drive. Ensure you bring your pet and reference number for admission. As part of this initiative, Tales of Tails, in collaboration with Rooikoppies Animal Hospital, will establish a satellite surgical unit on-site at The Blue Collar Pet Shop. Upon admission, your pet will be transferred to the surgery area where all necessary procedures will be performed. These will be conducted by Dr. K. Koetsee (BVSc) from Rooikoppies Animal Hospital. Following surgery, your pet will be placed in a monitoring ward at the pet shop. Once recovery monitoring is complete, you will receive a notification with the collection time. Please note: Overnight stays are not required. All pets are to be collected on the same day of the procedure.

I understand that there are inherent risks associated with the use of anaesthesia. If any complications arise after surgery, I will notify Tales of Tails immediately.Should my animal(s) be admitted to any other veterinary facility after the surgery, Tales of Tails, Rooikoppies Veterinary Clinic or any other veterinary practice used on the day, as well as The Blue Collar Pet Shop Lenasia, shall not be held liable for any accounts, medical costs, or related consequences.If my pet is in heat, I understand that an additional fee of up to R150 will be applicable. In the case of late-stage pregnancy, an additional charge of R200 for dogs and R100 for cats will apply.Any other conditions not listed above may result in additional charges. I understand that I will be informed by Tales of Tails in such cases.I, the undersigned and legal owner of the above-mentioned pet(s), acknowledge and accept all risks, including those associated with vaccine-preventable illnesses. All veterinary practices involved, as well as Tales of Tails and The Blue Collar Pet Shop Lenasia, will take all reasonable precautions to prevent the spread of disease. However, it is strongly recommended that all pets are fully vaccinated prior to being entrusted into their care.I hereby give my consent for Tales of Tails to transport my pet(s) to and from Rooikoppies Veterinary Clinic or any other veterinary facility they may choose to use. I understand and accept that Tales of Tails, the veterinary practice used, and The Blue Collar Pet Shop Lenasia cannot guarantee the outcome of any procedure or treatment, and I therefore indemnify all the above-mentioned parties against any adverse outcomes or unforeseen events.Any additional costs incurred on the day of treatment will be paid in full on the same day.I further agree and confirm that by proceeding with the booking, I automatically consent to being added to a WhatsApp group for communication on the day of the booking. I understand that my name and contact number will be visible to other members of the group for coordination purposes.This form shall be deemed valid whether I have read it or not, and shall be enforceable once my pet(s) has been picked up or once I have agreed to the collection — whether such agreement was written or verbal.
