Facility Group Registration
This form is designed for one person to register multiple individuals from their facility. Each registrant will receive confirmation and instructions for accessing the sessions.

Upon completion of this form, the contact person will receive an invoice from Square our payment processor. If you wish to include the Contact person in the billing, please complete the fields below and check the box to include the contact person. This invoice will be payable with a credit card. Please comment if a separate emailed invoice is required rather than that sent from Square.
