  • Dear Parent/Guardian:

    Welcome to the scholarship application for the Fall 2025 Young Actors Program at Studio Acting Conservatory.

    Please complete this application form as best as you can and submit it as soon as possible.

    Scholarship auditions will be held Saturday, September 13th by appointment. These auditions will help determine eligibility for our scholarships and help us place your student into the class that suits them best. Your student must be accompanied by a parent or guardian at the time of the audition. Please take a moment to carefully read over the program expectations in the next section. It is our priority to ensure we are creating the best and safest learning environment for your student. We expect parents and guardians to abide by the expectations to the best of their ability so as to not only support their student's experience, but the experiences of their classmates as well.

    Prior to receiving this application, you were required to pay a fee of $25.00. This application fee is non-refundable regardless of the scholarship decision or your student's participation in the program. If you receive a partial scholarship, this fee will not be applied the tuition balance owed.

  • Program Expectations

    • Students are required to attend each class.
    • Students must contact their teacher if they are unable to attend class. 
    • Students are required to be on time for each class. 
    • Students are required to participate during class.
    • Students are encouraged to dress in comfortable clothing for greater freedom of movement.
    • Classrooms and hallways are reserved for students’ use. Family members and/or friends are invited to wait in the Student Lounge.
    • Students must be able to attend and participate in the Final Scenes at the end of the semester, on Saturday, December 13th.
    • The safety and well-being of our students are our top priority. As with sports, acting has physical demands. Students are encouraged to consult their physicians regarding their ability to participate.
    • Students are expected to carry their own accident and medical insurance. Studio Acting Conservatory assumes no liability for injuries or damages resulting from class participation. 
    • Students may not engage in disruptive behavior or inappropriate conduct during class. 
    • Students may not deface or destroy the facilities. 
    • All Studio Acting Conservatory students, faculty, and staff must be vaccinated against COVID-19 in order to participate in Studio Acting Conservatory programming. 
    • All Studio Acting Conservatory students, faculty, and staff must adhere to the most recently updated COVID-19 safety protocols as set forth by the Executive Director.
    • Any student who violates the program expectations of the Studio Acting Conservatory will be discharged after incurring three warnings. Students who engage in gross misconduct will be immediately and permanently discharged.

     

    DISCHARGE POLICY

    A student who is discharged from the program due to repeated disruptive or dangerous behavior, excessive absenteeism, or excessive lateness will not be eligible for tuition or application fee refund.

  • Health and Emergency Information

    The information in this section is gathered to assist us in identifying appropriate care and to obtain emergency contact information
  • In case of emergency, I authorize the staff of Studio Acting Conservatory to use their best judgment in allowing my child to receive emergency medical or surgical treatment if necessary. I understand that every effort will be made to contact me prior to such an action. (Please be advised that it is imperative that your child be in good health when arriving for class. The duties of faculty do not include providing medical care.)

    I hereby certify that, to the best of my knowledge, the medical information is complete and correct. I agree to allow Studio Acting Conservatory staff to use their judgment in obtaining necessary medical care at the expense of the parent/guardian.

    This section should be completed by the student. The information compiled in this section will not be shared outside of the Conservatory Staff. Submitting information to this section is optional.
  • Financial Need Information

    This section should be completed by the parent/guardian. Studio Acting Conservatory is proud to reserve half of the placements in the Young Actors program for scholarship recipients. Because we have a limited number of scholarships to offer, we appreciate you sharing some information with us. Your completion of the following information will ensure that scholarships will be granted to the students with the most demonstrated need. Thank you!

  • Please submit a photo of your child. During the audition process, this photo will be used to keep track of which students are being placed in each class. This can be a normal photo taken with your cell phone. Please ensure your child's face is clearly visible and the photo is taken straight on.

  • I, the undersigned, hereby grant permission to Studio Acting Conservatory, any publishers or other interested media, to photograph my child and to record my child's voice, performances, poses, acts, plays and appearances, and to use my child's picture, photograph, video, silhouette and other reproductions of my child's physical likeness and sound for any nonprofit uses, promotion or advertising of Studio Acting Conservatory and the Young Actors Program and the unlimited distribution, advertising, promotion, exhibition and exploitation by any method or device now known or hereafter devised in which the same may be used, and/or incorporated and/or exhibited and/or exploited.

     

    I agree that I will not assert or maintain against you, your successors, assigns and licensees, any claim, action, suit or demand of any kind or nature whatsoever, including but not limited to, those grounded upon invasion of privacy, rights of publicity or other civil rights, or for any other reason in connection with your authorized use of my child's physical likeness and sound by Studio Acting Conservatory and the Young Actors Program as herein provided. I hereby release you, your successors, assigns and licensees, and each of them, from and against any and all claims, liabilities, demands, actions, causes of action(s), costs and expenses whatsoever, at law or in equity, known or unknown, anticipated or unanticipated, which I ever had, now have, or may, shall or hereafter have by reason, matter, cause or thing arising out of your use as herein provided.

     

    I affirm that neither I, nor anyone acting for me, gave or agreed to give anything of value to any of your employees or any representative of any television station, network or production entity for arranging my appearance in the advertising or promotional materials of Studio Acting Conservatory and the Young Actors Program.

     

    I have read the above and fully understand the meaning and effect thereof and, intending to be legally bound, I am signing this release. As all young actors are under 18 years of age, the parent or guardian is signing this release on their behalf.

  • Withdrawal Policy

    • The $25 application fee is non-refundable. 
    • If your Young Actor needs to withdraw from the program, the parent/guardian must inform the office in writing. Withdrawal requests should be sent to info@studioactingconservatory.org.
    • If you receive a partial scholarship, any tuition paid beyond the $25 application fee will only be refunded if the withdrawal and refund request are submitted in writing prior to or within three (3) business days of the first class meeting,
    • No refunds will be issued after the third business day following the start of class, regardless of whether or not the student is in attendance.

    By submitting, I attest that I have read and understood the withdrawal policy.
