CLIENT CONSENT: I, {name}, affirm that I am the legal owner of the pet(s) listed in the following patient form(s) provided to Neighborhood Vet. I further declare that I am authorized to make decisions regarding the health, treatment, and care of the pet(s) I submit under my account. I am 18 years old or older. I hereby authorize the veterinarians of Neighborhood Vet to examine, prescribe for, and treat the pets as described on the subsequent patient form. I understand that inherent risks exist with medical procedures, treatments, vaccinations, and medications, and I am encouraged to discuss any concerns I have about those risks with my veterinarian before the procedure(s) are initiated. I will not hold Neighborhood Vet Clinic, the veterinarians, or any staff member liable for any complications that may arise. My signature on this consent form indicates that no warranty or guarantee has been given to me as to the results or cure afforded by the recommended treatments or procedures. I agree to pay for all services rendered and medications, goods, and supplies when purchased. ALL FEES ARE DUE AT THE TIME SERVICES ARE RENDERED. I understand that any outstanding balances may be subject to applicable late fees and collection costs.
