Tennessee Residency Requirement
Due to state laws and regulations, we are only permitted to provide treatment to individuals who reside in Tennessee.

To qualify for treatment, you must possess either:

A driver's license or state-issued photo ID from Tennessee
- or -
A driver's license or state-issued photo ID from another state (or U.S. territory) along with supporting documentation.

Do you possess a valid driver's license or government-issued photo ID?*

You may still qualify for treatment if you posesses:

A valid driver's license or state-issued photo ID (from any U.S. state or territory)
One or more of the following documents that includes your name and a current Tennessee address:
- A lease or mortgage statement
- A recent utility bill
- Official U.S. mail, such as a bank statement, insurance statement, or government correspondence.
- Other document that reasonably establishes Tennessee residency.
