St. Basil Baptism Info Form
  • St. Basil Baptism Info Form

Baptisms are scheduled most Sundays at 12:00 pm Noon. Detailed Baptism Prep timelines are available for parents Baptizing
  • We're so happy you are bringing your child into God's family, the Church!  We are here to make this special celebration beautiful and sacred.

  • Parent Information

  • Eligibility to be a Godparent (Sponsor)

    Please read carefully
  • A person to be Baptized can have 1 or 2 Godparents.  If there is to be only 1, then it can be a man or woman (Godfather or Godmother).  If there are to be 2 Godparents, then one must be male and one must be female, but at least 1 of the Godparents must fit ALL of the following criteria to serve as a Godparent in the Catholic Church:

    • Be a Catholic who has received the Sacraments of Baptism, First Holy Communion, and Confirmation
    • AND, a person who faithfully practices and believes in the Catholic faith, attends Mass, and is a participating member in good standing of a parish community
    • AND, a person who is at least 16 years of age
    • AND, not be the mother or father of the child to be Baptized
  • Eligibility to be a Christian Witness

Not necessary, but 1 is allowed if there is already a faithful Catholic acting as godparent.
  • If you are already planning to have 1 Catholic Godparent who meets all of the criteria above, a baptized Christian from a non-Catholic ecclesial community may serve as a "Christian Witness".  They are not a Godparent but are welcome to be a Christian Witness.  The Christian Witness must be a Christian Baptized with the valid Trinitarian formula and intention (most Christian communities fit this description but you may check with your priest or deacon).

  • Since this is the first child you are Baptizing, you will view 3 videos on your own and fill out a short packet that corresponds to the videos.  After you have viewed the videos and filled out the packet, email Deacon Rob Blackburn at rblackburn@basilthegreat.org to schedule a time to meet with him.  Bring the completed packet with you to the meeting with him.

Here is a link to the videos and packet, but be sure to come back and finish submitting this form by hitting Submit below.
