Please note that signing up for the workshop is not the same as registering to audition. Many students may choose to attend the workshop and decide later whether to register for an audition.

I authorize Falcon Footlights to provide my child's teachers with a list of those registered through this form. For students from all other schools, parents are responsible for transportation to and from the workshop. I agree to pick up my child at 5:30 p.m. in the front drive of Farmington Elementary School.*
