Acknowledgment of Understanding
By signing this document, I acknowledge that I have read and understood every section of this waiver. I understand that participation in this event is entirely voluntary and that I am free to decline participation at any time prior to the start of the event.
Assumption of Risks
I understand that participation in flag football and related activities involves inherent risks, including but not limited to:
Collisions, falls, and physical contact with other participants
Muscle strains, sprains, broken bones, concussions, or other bodily injuries
Exposure to extreme weather, uneven playing surfaces, or field hazards
Loss, theft, or damage to personal property
Illness or disease transmission, including but not limited to COVID-19
I knowingly and freely assume all risks, whether known or unknown, even if arising from the negligence of the organizers or others.
Release of Liability
In consideration for being allowed to participate, I fully release and discharge Chozen Sports, its owners, directors, employees, volunteers, event sponsors, event charities, and all associated representatives (collectively, “Organizers”) from any and all liability, claims, demands, or causes of action arising from participation in this event, including those caused by negligence, to the fullest extent permitted by law.
This release applies to injuries, illnesses, death, property damage, or other losses sustained as a result of participation in this event.
Indemnification
I agree to indemnify and hold harmless the Organizers from any costs, damages, or legal fees arising from any claim related to my participation, whether brought by me or a third party.
Eligibility
I understand that the Organizers are not responsible for determining or verifying my eligibility to participate. I certify that I am physically fit, have not been advised against participation by a qualified medical professional, and have no conditions that would pose an undue risk to myself or others.
Media Release
I grant full permission for the Organizers to record, photograph, film, or otherwise capture my participation in the event and to use my likeness in any media (including print, broadcast, and online) for publicity, promotional, advertising, or commercial purposes without compensation or the need for further approval.
COVID-19 Acknowledgment
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk of exposure or infection. I agree that the Organizers shall not be held responsible for any illness related to COVID-19 or other communicable diseases.
Severability
If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Binding Agreement
This document is binding upon me, my heirs, executors, administrators, and assigns, and shall be governed by the laws of the State of North Carolina.
