    To qualify for treatment, you must possess either:

    • A driver's license or state-issued photo ID from Tennessee
      - or -
    • A driver's license or state-issued photo ID from another state (or U.S. territory) along with supporting documentation.

    • One or more of the following documents that includes your name and a current Tennessee address:
      • A lease or mortgage statement
      • A recent utility bill
      • Official U.S. mail, such as a bank statement, insurance statement, or government correspondence.
      • Other document that reasonably establishes Tennessee residency.
