WAIVER AND RELEASE AGREEMENT
By agreeing, I give permission for my child to participate in Blue Mountain Christian University Camp(s). I recognize these activities will involve intense activity and certify that the participant is fit to engage in activity of this sort. I recognize the inherent risks involved with athletics camps can include, and not limited to, sprained ankles, sprained knees, fatigue, soreness, broken bones, concussions, cramps and other inherent risks involved with athletic training. In case of an emergency involving the participant, I understand that the camp staff will make all reasonable attempts and efforts to contact me at the supplied telephone number. If I am unable to be contacted, I hereby authorize the camp staff to secure proper treatment. I give my approval for participation in Blue Mountain Christian University Athletic Camp(s) on the campus of Blue Mountain Christian University and assume all risks and hazards incidental to such participation, and I waive, release, indemnify and forever agree to hold harmless Blue Mountain Christian University, its staff, officers, administrators, agents, representatives, sponsors, advertisers, board members, coaches, teams, volunteers and employees.
COVID-19 AND OTHER CONTAGIONS
I am fully and personally responsible for my child’s safety and actions while and during their participation, and I recognize they may be, in any case, at risk of contracting COVID-19 or other contagions. With full knowledge of this, I also hereby release Blue Mountain Christian University, its staff, officers, administrators, agents, representatives, sponsors, advertisers, board members, coaches, teams, volunteers and employees from any and all liabilities arising out of or related to any loss, damage, injury or death that may be sustained by the camper related to COVID-19 or other contagions while participating in any activity while in, on or around Blue Mountain Christian University premises, or while using the facilities that may lead to unintentional exposure or harm due to COVID-19 or other contagions. I agree to indemnify, defend and hold harmless Blue Mountain Christian University from and against any and all costs, expenses, damages, lawsuits and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss or death from or related to COVID-19 or other contagions.
PHOTO RELEASE WAIVER
I hereby grant Blue Mountain Christian University permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos will become the property of Blue Mountain Christian University and will not be returned.
I hereby irrevocably authorize Blue Mountain Christian University to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge Blue Mountain Christian University from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE AGREEMENT. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY CLICKING THE “SUBMIT” BUTTON BELOW.
