Staten Island USBC Hall of Fame Nomination Application
To be considered, candidates MUST meet the following requirements by August 1 of the current application year:
1. Be at least 40 years of age.
2. A member of USBC (formerly ABC/WIBC) for a minimum of 20 years. a maximum of 5 years as a youth member may be substituted.
3. Been a member of a Staten Island certified league for at least 20 years.
Application Deadline: February 1 of the current year.

Note: The Staten Island USBC Hall of Fame Selection Committee may waive some requirements when considering a candidate for election posthumously.

Evaluation and nomination of candidates to the Staten Island USBC Hall of Fame will be made by the Hall of Fame & Awards Selection Committee. Candidates must have excelled in or made outstanding contribution to the sport of bowling within the jurisdiction of the Staten Island USBC.

There are two categories for election into the Staten Island USBC Hall of Fame:
- Superior Performance
- Meritorious Service
Applications must be received by the SIUSBC Hall of Fame Selection Committee on or before February 1st of the current year for consideration.

No application received after this date will be considered in the application year. Candidate applications received but not forwarded for nomination will be retained and may be considered in subsequent years. Applications will be retained by the SIUSBC for a period of 5 years.

In seeking nomination to the Staten Island USBC Hall of Fame, the requested information in Part I (Superior Performance) or Part II (Meritorious Service) must be furnished, with all substantiating documentation. Missing information will result in disqualification from consideration.
