Let’s recognize the outstanding team members who truly embodied our H.I.R.E.S.(Honesty, Integrity, Respect, Excellence, and Service) values this quarter! To ensure fairness and meaningful recognition across the territory, please take a moment to reflect and provide specific examples of impact. Submissions with clear, thoughtful examples will be prioritized in the final selection process.

Revised Instructions:

1. The team member does not have to work in your department or location.
2. This online form does not apply to PWI. They will continue to use their current process.
3. See your supervisor if you prefer the current paper form.
4. Team members who do not win may have their name submitted again.
5. Winners are not eligible to win again for one year after receiving the award.
6. People Services team members are eligible to nominate or be nominated. 
7. Strategic Leadership team members and Good Life Manager are not eligible to be nominated.
