Parental Consent for Participation

I consent to let my child participate in Fish Lake Faith Formation. It is understood that every precaution will be taken for the safety and well-being of my child, but in the event of accident or sickness, Fish Lake Lutheran, its staff and its volunteers are hereby released from any liability.

I consent to let my child be photographed. These images may be shared with the Fish Lake community. (Note: We do not publish the last names of children.)
