WAIVER AND RELEASE OF LIABILITY:

Please read carefully as all participants must complete this form to participate in the attractions offered at City Jump Family Entertainment Complex.
In consideration of being allowed to participate in any activities or use any equipment or facilities at City Jump, the undersigned acknowledges and agrees to the following terms:

1. Acknowledgment of Risk: I understand that participation in activities at City Jump involves inherent risks, including but not limited to falls, collisions, physical exertion, and the use of equipment. These activities may result in physical or emotional injury, paralysis, permanent disability, or death. 2. Assumption of Risk: I voluntarily agree to participate (or allow my minor child to participate) in the activities at City Jump and assume all risks of injury or harm that may occur, whether arising from the negligence of City Jump, its employees, contractors, or otherwise. 3. Release of Liability: I release, discharge, and hold harmless City Jump, its owners, officers, employees, contractors, agents, and representatives from any and all claims, liabilities, damages, injuries, or losses (including attorney’s fees) arising from my (or my child’s) participation in activities at City Jump. This release extends to claims of negligence by City Jump or others. 4. Indemnification: I agree to indemnify and hold harmless City Jump from any claims made by others arising out of my (or my child’s) actions while on the premises or participating in activities. 5. Medical Treatment Consent: In the event of an emergency, I consent to emergency medical care and transportation, understanding that I am responsible for any associated costs. 6. Photographic Release: I grant City Jump the right to use photos or videos taken of me (or my child) during activities for promotional or marketing purposes without compensation. 7. No Supervision Guarantee: I understand that City Jump does not provide continuous supervision and that I am responsible for supervising my child while on the premises. 8. Compliance with Rules: I agree to follow all safety rules and instructions provided by City Jump staff and to ensure that my child complies as well. Failure to comply may result in removal from the facility without refund. 9. Severability: If any part of this agreement is found to be unenforceable, the remaining provisions shall remain in effect.
