Motivus Laser Tag – Release of Liability, Waiver of Claims, and Assumption of Risk Agreement
In consideration of being allowed to participate in any way in Motivus Laser Tag events or activities, I, the undersigned participant (or parent/legal guardian if under age 18), acknowledge and agree to the following:
1. Assumption of Risk
I understand and accept that participation in laser tag and related activities involves physical exertion and potential risks, including but not limited to slips, falls, collisions, and exposure to environmental hazards. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of Motivus Sports LLC or others.
2. Release of Liability
I voluntarily release, discharge, and agree to hold harmless Motivus Sports LLC, its owners, officers, employees, agents, affiliates, and volunteers (the "Released Parties") from any and all liability, claims, demands, losses, or damages — on account of injury, disability, death, or property damage — arising from or related to my participation in laser tag activities, whether caused by negligence or otherwise.
3. Waiver of Lawsuit
I agree not to sue the Released Parties for any injury, loss, or damage arising out of or related to participation in Motivus Laser Tag events. I understand that this agreement bars me and anyone acting on my behalf from pursuing any legal action against the Released Parties.
4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities (including attorney’s fees) brought as a result of my involvement in Motivus Laser Tag activities.
5. Fitness to Participate
I certify that I (or my child) am physically fit and have no medical condition that would make participation unsafe. I will follow all safety instructions and use equipment properly.
By signing below, I confirm that I have read and understood this agreement, and that I am signing it voluntarily. I understand this is a legally binding release of liability and waiver of legal rights.
