This form is an enquiry, please use it if you're looking to get booked in or just a quote.

We are Irezumi Ink, in Blackpool, Lancashire, just outside the city centre. Approx 10 minute drive from the city centre.

The studio is NOT FULLY accessible. There are three steps leading to the front door, we do have a ramp but this also has a drop down step. Inside the studio everything is fully accesible.

Please keep in mind our general styles.

We do not tattoo anything offensive, racist, homophoic, transphobic etc.

We will tattoo:

Linework
Fineline at a good size
Anime/Manga
Solid blackwork
Traditional with traditional colours
Text, words or symbols
Gothic/horror
Black and grey realism
Portraits
Memorical designs

We usually do not send out custom designs before the appointment, this can vary depending on the artist. The design will not be complete and will be a linework drawing of your design, colour or shading will be incomplete.
Please can you tell us about your idea in as much detail as you can.

If the design you are wanting is taken, we can always redraw it for you to make something similar.

Flash designs are drawn with a size in mind or a set size especially for detailed pieces.

Do you have any medical requirements? (Someone else present, mobility needs or conditions I need to be aware of beforehand.
We cannot tattoo you if you have any of the following:
If you're pregnant
If you are on strong blood thinning medication
Taking antibiotics (if you're already fighting infection we would say please wait until you're off the medication)

Please check with your GP if you are on any medication for a condition. eg. immunosuppressants, if you are diabetic or have any other issues you are unsure if it's safe to be tattooed.

We will never discriminate against a condition so please be as honest as you can filling out our form so we can best look after you.

You will be contacted to confirm the details of the appointment.

Please read the below info before finalising your booking request.

All bookings require a deposit, if you dont send a deposit within the dicsussed time frame the appointment time or design may be offered to someone else.

All deposits are 100% non refundable after you send them for any reason.

Please let us know if you need to rearrange or cancel your appointment at least 48hr before your appointment, failure to do thuis will result in a forfeit of your deposit.

Deposits are between £20-£100.

Deposits always come off the total on the day.

If you are more then 30 mins late your deposit (without good reason) will be forfeit. Please let me know ASAP if you're going to be late as it effects oour other appointments.

If you no show you will only be able to book in with us again by paying for the whole appointment in full.

If you wish to rearrange your appointment more than once we will ask for another depsoit if you wish to book in for a second time.

Please eat a good meal before your appointment and please bring a sugary drink and sweets.

Please do not consume alcohol or drugs 24hr before your appointment, not including your regular medications such as antidepressants ect.

Please bring photo ID, tattoos are 18+. This is the law, no exceptions.

Touch ups

If you require a touch up please contact tthe Artist directly.
I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up and will be required to send a deposit to confirm the appointment. I understand that if I make changes to my design or booking after submitting this request, the appointment cost may vary from the quote I was given and may be rescheduled if there is not adequate time for the artist to prepare.*

I understand that after I send my deposit it is 100% non refundable for any reason. deposits will only be transferred once for the same booking. if you wish to CANCEL, reschedule (more than once0 your deposit is forfeit and I will need a new deposit. I require 72 hours notice to reschedule an appointment.*
