Waiver and Code of Conduct Agreement

IMPORTANT: THIS IS A LEGAL DOCUMENT. READ CAFEFULLY BEFORE AGREEING.
In consideration of being permitted to participate in the 3 On 3 Basketball Tournament (the "Event") hosted by Victory Chapel, I, the undersigned participant (or parent/legal guardian if under 18), hereby agree to the following:

Assumption of Risk
I acknowledge that participation in basketball involves inherent risks, including but not limited to collisions, falls, overexertion, ball impact, physical contact, sprains, fractures, concussions, heat-related illness, cardiac events, and even death. These risks may result from the actions, omissions, or negligence of myself or others, including but not limited to other participants, spectators, event organizers, volunteers, referees, or facility staff. I voluntarily choose to participate in this Event, and fully assume all risks, whether known or unknown, foreseeable or unforeseeable.

Health and Medical Condition
I certify that I am physically able to participate in basketball activities and have no known condition that would prevent or increase risk during play. I will inform the event organizers of any medical condition or medication that may affect my participation or require emergency care. In the event of illness or injury, I authorize emergency medical care and agree to assume any costs arising from such treatment. I understand that no medical insurance is provided by the event organizers.

COVID-19 and Communicable Disease Clause
I acknowledge the contagious nature of COVID-19 and other communicable diseases and voluntarily assume the risk of exposure, infection, or other illness from participation. I release the event organizers from any liability related to illness caused by participation in the Event.

Release and Waiver of Liability
To the fullest extent permitted by New york law, I hereby release, waive, discharge, and hold harmless Victory Chapel, its directors, officers, volunteers, agents, employees, sponsors, and facility owners, from any and all claims, liabilities, demands, causes of action, costs, or expenses of any kind, including attorneys' fees, arising out of or relating to personal injury, illness, disability, death, or property damage related to participation in the Event, even if caused by negligence.

Indemnification
I agree to indemnify and hold harmless the released parties from any claims made against them by third parties (including other participants or family members) arising out of my actions or participation.

Photo and Video Release
I grant permission for Victory Chapel to record, photograph, or film my participation and to use such materials in promotional or informational publications, including social media and websites. I waive any rights of compensation or ownership.

Severability and Jurisdiction
This Waiver is intended to be as broad and inclusive as permitted by the laws of the State of New york. If any part is held invalid, the remainder shall continue in full legal force. Any legal action arising from this agreement shall be brought in New York courts, and I consent to jurisdiction therein.
