Holter Monitor Rental Agreement & Liability Waiver

Rental Fee: $120 (plus shipping) Alba Reading Fee: Paid directly by Renter to Alba Medical

 

By reserving a Holter monitor from Goldpeak Dobermans, the Renter agrees to the following

1. Equipment Condition & Return The Holter monitor and all included accessories are provided in working condition. The Renter agrees to return all items by the agreed date in the same condition they were received.

Shipping: Renter is responsible for all shipping costs (to and from)

Rental Period: _____ days (unless otherwise arranged.) 

Late Fee: Additional daily charge may apply if returned late

3. Responsibility for Damage or Loss The Renter assumes full responsibility for the monitor and accessories.

Replacement Fee (if lost or irreparably damaged): $1,000

4. Alba Medical Reading Fee The Renter understands that they are responsible for submitting the Holter data to Alba Medical and paying their fee directly. This fee is not included in the rental price. If Renter wants Provider to submit the results to Alba, please let it be known.

Monitor must be used according to instructions provided

Device should never be left unsupervised on the dog, dog needs to be under constant supervision

Keep away from children, other pets and water

6. Medical & Legal Disclaimer This Holter monitor is a screening tool only. It is not a substitute for veterinary care or diagnosis. 

Goldpeak Dobermans is not liable for any injury, stress, or health outcomes related to the useor misuse of the monitor.
