KidWorks Service Agreement

Welcome to KidWorks! We are pleased that you have chosen to provide KidWorks with your services. Community Partners, Volunteers, and Service Providers are an important and valued part of KidWorks. We hope that you enjoy working with us and feel like a part of our team. By signing this Service Agreement, you agree to abide by our policies and perform only the services agreed to by KidWorks.
Service Agreement

I understand that I represent KidWorks as a participant in activities hosted by or in benefit of KidWorks. I agree to perform my services to the best of my ability; to follow the organization’s policies and procedures, including health and safety, in relation to its staff, volunteers, clients and partners. I will remain professional throughout the event/program I am participating in.

Liability Waiver

I understand that if I am injured in the course of the activity, I am not covered by KidWorks’ workers compensation program. I authorize KidWorks to seek emergency medical treatment on my behalf in case of injury, accident or illness to me arising from my involvement as a volunteer or service provider.

I understand that I will be responsible for medical costs incurred by such accident, illness or injury. I hereby release and discharge KidWorks and its officers, employees, agents, and contractors from all actions, claims, or demands that I, my heirs, guardians, and legal representatives now have, or may have in the future, for injury or damage resulting from my participation in the project. KidWorks is not responsible for the loss or damage to any personal possessions and property.

Communications

I give full permission to KidWorks, its representatives and/or employees the right to photograph, film, audio record, televise, and/or disseminate information and content of me, and/or my property for publicity purposes.
