PERMIT TO LEAVE SCHOOL

ARVIDA MIDDLE SCHOOL
- Remain outside of the building at the Main Entrance.
- This form must be completed AT THE TIME the student is being signed out and NOT prior to arrival at the school.
- This form must be submitted by 3:10 pm for pick-up
- Once the form is completed, PLEASE WAIT a few minutes and then call the Main Office at 305-385-7144 and provide Student's Name.
- Once authorized, an employee will escort the student to the Main Entrance.
Only those authorized individuals listed on the 2025-2026 Emergency Student Data Form as completed by the registering parent/guardian may sign a student out.

Picture ID must be presented for verification at time of pick up.
