4.) Authorization for Third-Party Assessment
During the school year, DCP may wish to retain third-parties to perform observations and non-invasive assessments in order to gather information about a child's health and development, abilities, and behaviors to assist in better planning and support for meeting each child's individual needs. These third parties will use any information gathered during these assessments solely to assist DCP, the child, and the family. For example, DCP may retain third parties to assess a child's speech development and/or to observe a child's abilities and behaviors in the classroom.
DCP or any of its staff will not arrange these observations or assessments without first discussing their concerns with the parent/guardian of the child in question.
I give my permission for DCP to retain third-parties to assess my child during the school day, for DCP to disclose any relevant information to such third-parties, and for such third parties to provide, whether verbally or in writing, the results of their assessments to DCP (including but not limited to the Executive Director or my child's teachers) and to me. DCP assumes no liability and is hereby released from liability of any nature in connection with such assessments.
