    This report form is to record witness accounts of incidents that have occurred at CBBA sanctioned events (i.e. domestic comp, Aussie Hoops, Reps, etc.).
  • What is an Incident Report?

     
    An Incident Report is an official record of any event that occurs during CBBA competitions, training sessions, or events that involves a rule violation, injury, misconduct, or any situation requiring further review or action.

     
    How is an Incident Report Used?

    • Documentation: It ensures that all relevant details of an incident are recorded accurately, including the people involved, what happened, and any immediate actions taken.
    • Review & Investigation: CBBA may use the report to review the incident, determine whether further action is required, and apply appropriate policies, such as the Zero Tolerance Policy or BNSW Disciplinary Tribunal By-Laws.
    • Safety & Accountability: It helps maintain a safe and fair playing environment by identifying potential risks, ensuring appropriate follow-ups, and preventing future issues.
    • Disciplinary Process: If necessary, the information in an incident report may be referred to CBBA management or a tribunal for further action, including warnings, suspensions, or other sanctions.


    Who Should Complete an Incident Report?


    Incident reports can be submitted by referees, supervisors, coaches, players, or spectators who witness or are directly involved in an incident. Reports should be completed as soon as possible after the event to ensure accuracy.

    How to Report an Incident Appropriately

     
    For an Incident Report to be most effective, it should be clear, factual, and objective. The report may be used for investigation, disciplinary review, or tribunal hearings, so it is important to provide accurate and detailed information.

     
    Best Practices for Reporting an Incident:

     

    1. Stick to the Facts
      • Describe exactly what happened without personal opinions or assumptions.
      • Avoid exaggeration or speculation, only include what you saw or heard.
    2. Be Specific and Detailed
      • Include who was involved, what occurred, when, and where the incident happened.
      • If applicable, mention any actions taken immediately after the incident.
    3. Use Clear and Concise Language
      • Write in complete sentences and use neutral, professional language.
      • Avoid emotional or vague statements, state what happened as clearly as possible.
    4. Include Witnesses (if any)
      • If there were other people who saw or heard the incident, include their names and roles (e.g., referee, coach, spectator).
      • Witness accounts can provide additional perspectives and support the report's accuracy.
    5. Submit as Soon as Possible
      • Complete and submit the report immediately or as soon as possible after the incident to ensure accuracy and reliability.

    Items to include (but not limited to): 

    • Names/Jersey Numbers.
    • Location.
    • Game time and date.
    • Team Names.
    • Positions (coach, player, spectator, referee, etc.). 
    • Detailed report of what happened:
      • What was said. 
      • What actions occurred. 
      • Etc. 
    • Any footage, recordings, etc. 
    • Other witnesses that can attest to your version of events. 

     

    For Online Incidents: 

    • Screenshots of messages.
    • Saved voice messages.
    • Etc.

    The overall aim of wirting an incident report is to give CBBA an opportunity to investigate these incidents thoroughly and fairly. Any report that lacks critical information will not be used.
