  • Financial Responsibility
    Copays are due at the time of your visit. As a courtesy, we do our best to estimate your copay, but please remember that your insurance is a contract between you, your employer, and your insurance company. Any claims not paid by your insurance will be your responsibility. Not all services may be covered by your plan.
    If your insurance changes, please inform us at least 48 hours before your appointment. This helps us verify your new benefits and avoid unexpected out-of-pocket costs.

    Missed Appointments & Cancellations
    Your appointment time and materials are reserved just for you. If you need to cancel or reschedule, please give us at least 48 hours' notice to avoid a $50 cancellation fee.
    After three missed or late-cancelled appointments, we may ask you to transfer your care to another office that may better suit your schedule.

    Appointment Confirmations
    We make every effort to confirm appointments. If we are unable to confirm your appointment, it may be removed from the schedule to accommodate other patients in need of care.
    Please make sure to confirm your appointment to keep your spot.
