    1. Federation or Umbrella Organization (SELECT IF YOU ARE APPLYING ON BEHALF OF MULTIPLE ORGANIZATIONS) – An organization registered as a nonprofit organization in Colorado by obtaining a Certificate of Authority from the Secretary of State’s office and exempt from federal income tax under Internal Revenue Code 501(a) as described in Section 501(c). This organization serves as the agent for a group of at least seven (7) nonprofit organizations and otherwise meets the criteria established in the Bylaws of the Colorado State Employee Advisory Committee on Charitable Giving.
    2. Colleges & Universities/State Service Organization, or Non-Federation Nonprofit (CU/SSO+) (SELECT IF YOU ARE APPLYING ON BEHALF OF A SINGLE ORGANIZATION) – A CU/SSO+ is not a member of a federation or umbrella organization but is either:
      1. Directly associated with a Colorado department or an institution of higher education and otherwise meets the criteria established in the Bylaws of the Colorado State Employee Advisory Committee; OR
      2. A nonprofit organization that is not a member of a federation or umbrella organization nor a CU/SSO but can clearly document its service to the State of Colorado or its community and otherwise meets the criteria established in the Bylaws of the Colorado State Employee Advisory Committee.

    The above listed application fee will receive an invoiced via email to the listed application contact email within 24 to 48 hours of application submission.

    My organization understands the application fee is non-refundable under all circumstances and agrees to pay the total application fee amount listed above within 15 days of the CCC application deadline, Friday, May 29, 2026.

  • Organizational Information & SoS Registration

    Nonprofit Informational Worksheet
  • All applicants must complete and upload the Nonprofit Informational Worksheet, which is used to capture applicant information such as Legal/DBA name, Federal EIN, address, financial reporting, state registration information, service categories, and more.  

    If you do not have a copy of the Nonprofit Informational Worksheet, CLICK HERE to download a copy. 

    *Note: the form must be downloaded and opened in excel to display properly.*

    Both Federation/Umbrella and CU/SS0+ applicants will complete the same worksheet.

    Federations/Umbrella applicants will use the worksheet to provide organizational information for both the federation and its member charities, with each completed row in the worksheet representing an application.

    Federations will have to option to provide marketing materials for both the federation and member charities in the "marketing materials" sheet.

    CU/SS0+ applicants will only need to complete the first row of the worksheet to provide their organizational information. 

    CU/SS0+ applicants can provide their marketing materials directly in this webform. 

     

    The information from the worksheet will be imported into the CCC brochure, online resources and elsewhere. Be sureto carefully proofread the information before uploading the Excel file.

     

  • Secretary Of State Good Standing & Registration Status

  • All applicants will provide the below information by completing the "SoS Info for Nonprofits" tab located in the Nonprofit Informational Worksheet.

    Instructions for collecting and recording the requested information can be located in the Instructions tab of the Nonprofit Informational Worksheet.

     

    Business Good Standing Status

    1. URL to your organization's Good Standing business summary page on Colorado's Secretary of State website
    2. Most recent "Periodic Report number" under "Filing history and documents"
    3. Your organization's incorporation/establishment/formation
      date as listed on the business summary page

     

    Registration Statement for Colorado Charitable Organizations

    1. URL to your organization’s Registration Statement for Colorado Charitable Organizations with a “good” status via the organization summary page which can be found using the Colorado's Secretary of State chartiable organizations search page.
    2. your organization's most recent "Renewal ID" number found on the Registration Statement.

    • Note: organizations that do not directly serve the State or People of CO but maintain an undefined nexus to Colorado (EX: employ residents or manufacture/store goods within CO to support service provision) may be accepted at the discretion of the board.

  • Description of 990 requirements for the Colorado Combined Campaign (CCC)


    All CCC applicants must provide either IRS Form 990 or a Pro Forma 990 dated within the past 18 months from January 2026.

    • If your organization is not required to submit an IRS Form 990 to the IRS (organizations with annual revenues at or less than $50,000), a Pro Forma 990 must be provided.
    • The Pro Forma 990 is an IRS Form 990 that is completed only for CCC purposes. The IRS Form 990 can be downloaded from the IRS website. The following sections must be completed:
      • Page 1, Items A-M
      • Part I (Summary), Lines 1-4 only
      • Part II (Signature Block)
      • Part VII (Compensation - section A only)
      • Part VIII (Statement of Revenue)
      • Part IX (Statement of Functional Expenses)
      • Part XII (Financial Statements and Reporting) Line 1

  • Colorado-based agencies have a posted notice that summarizes the discriminatory or unfair practices prohibited by the law per Colorado Civil Rights Commission Rules (3 CCR 708- 1). Notice must include contact information for filing a complaint. 

    All agencies must provide a signed affirmation of non-discrimination with respect to employees, clients served, and volunteers consistent with all applicable local, state and Federal law.

    By uploading these documents, your organization provides permission for these items to be published on the CCC website, social media, and campaign brochure.

    While providing these materials is highly reccomended in order to maximize your organization's visibility to CCC donors, their provision is optional and will not impact the determination of your organizaiton's eligibility to participate in the 2026 CCC.
