Please complete this form to purchase Guelph Storm ticket voucher(s), as part of a fundraising initiative by the Alzheimer Society Waterloo Wellington (ASWW).
Each ticket voucher costs $25. Ticket vouchers can be used for any regular 2025-2026 season Guelph Storm home game (this does NOT include playoff games).

Ticket vouchers can be arranged to be picked up from the Alzheimer Society Waterloo Wellington Guelph office. The address is 202-25 Wellington St W, Guelph ON, N1H 7T9.

If you are unable to pick them up during office hours, please indicate below and we can arrange to drop them off to you.

E-transfer at the time of pick up is the preferred method of payment. Cash and cheque are also accepted.

Please do not drop into the office for ticket pickup.
