    • One representative for your company must be selected as the DocuSign Signatory contact. This is the individual who can execute contracts, purchase orders, and sign for other important documents. We require only ONE contact. If there are multiple individuals, we ask that your designated contact distribute the document for signatures internally in your organization as needed.

      Please provide the following: 

      1. Current completed W-9 form (update annually by January 31st)
      2. Current copy of your business license OR current contractor’s license, whichever applies.
      3. Current MWBE certificate, if applicable
      4. Project-specific SDS (Safety Data Sheets) information for all products
      5. If you are a subcontractor, the following is also required: a current blanket certificate of liability insurance to include the General Liability and Worker’s Compensation policy expiration dates.
