When mailing in your repair, please send ALL devices wrapped in bubble wrap and postal box to secure and protect during shipping.
For repairs under warranty, please provide a copy of your dated proof of purchase (receipt) either in the postal box, or upload your proof of purchase (receipt) to this form (At the bottom of the page).
*We recommend insuring your package and sending through a trackable service (USPS, UPS, etc.) in the unlikely event of loss or damage during shipping.*
