You’re invited to participate in our cluster map initiative for individuals who have a personal diagnosis or suspect a cluster of neurodegenerative brain diseases in their community. A "cluster" is defined as an unusually high number of individuals with the same condition within the same area.
The purpose of this initiative is to create a nationwide map of communities affected by Parkinson’s and related disorders in order to raise public awareness and help uncover potential environmental or geographic factors contributing to these diseases.
This brief online survey should take approximately 3 minutes to complete, and includes questions about your contact information, demographics, and health.
All responses you provide for this survey will be kept confidential. If the results of the survey are reported, you will not be identified by name or any other information that could be used to infer your identity.
