THE EMCEE WILL USE SOME OR ALL OF THE INFORMATION ON THIS FORM WHILE THE CONTESTANT IS ON STAGE.

TO AVOID MISPRONOUNCING NAMES, PLEASE INCLUDE ANY INFORMATION TO ASSIST THEM.

If accepted to compete, Contestant will need to be present Sept. 20th from 2:30 PM- 4:30 PM (times subject to change and competition may run longer) at Main Stage, as well as at The Main Stage between T.V.R 8:00-9:30 PM for the announcement of the winners, exact time will be announced as it gets closer.

Also, a little different this year, we are asking if the Contestants would go to TVR at Riverfront on the Friday night before and mingle with Bikers and get them to come vote for you the next day, Saturday the 19th. If you take a selfie with someone down there, you will receive 10 extra points during the contest.

Waiver of Liability: In consideration of your accepting this entry, I, the undersigned, intending to be legally bound, herby from myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against the City of Cottonwood’s Thunder Valley Rally, Main Stage and their representatives, successors, and assigns, for any and all injuries suffered by me in this event. Further, I hereby grant full permission to Main Stage, and the City of Cottonwood’s Thunder Valley Rally and/or agents authorized by them to use any photographs, videotapes, motion pictures, recordings and any other record of this event for any legitimate purpose.
