To Have and To Hold Questionnaire (Wife)
Your story matters. As a married couple, a widow, or a widower, we welcome you to share your experiences and the wisdom they hold. Family members may also participate by sharing their loved ones' marriage stories. All participants must meet the following criteria: *Complete the To Have and To Hold Questionnaire for both husband and wife. *Have been married for one year or more (spouse can be deceased). * At least one is a member of the AME Zion Church. Laity, clergy, bishops, officers, etc., are invited to participate. *Have a valid marriage license. *Grant consent from both members (or the surviving spouse) to have your story and photos published. *Grant consent to a virtual or in-person interview, if needed. ***NOTE: A family member may submit a couple’s story, but their story must meet the criteria stated above. The submitter must consent to have the story and photos published. Proof of relationship is required.
Instructions:
1. A survey for the wife and one for the husband (separate link) must be completed for each couple. (NOTE: For a widow/widower or family member submission, please complete only one questionnaire, but include names of both on the form, and your version of the story.) Please complete independent of one another. Thank you! 2. Please ensure the correct spelling of names in the first question box. 3. Please email up to five photos spanning your relationship and marriage. Photos may include your children, family members, friends, or other people who were instrumental in bringing the relationship together. 4. Please feel free to share with other members of The AME Zion Church. 5. The deadline for submissions is 11:59 PM PST, October 31, 2026.
