Applications should be submitted no less than 21 days before an event.
Before filling out the application please be aware of the following requirements.
Meet Announcement: Application for approval shall be accompanied by the meet announcement which shall include the following:
A statement of the nature of prizes to be awarded.
Information about water depth measured for a distance of 3 feet 31/2 inches (1.0 meter) to 16 feet 5 inches (5.0 meters) from both end walls.
The following statement: “In granting this approval it is understood and agreed that USA Swimming shall be free and held harmless from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event.”
One of the following two statements:
(1) The competition course has been certified in accordance with 104.2.2C(4). The copy of such certification is on file with USA Swimming; or (2) The competition course has not been certified in accordance with 104.2.2C(4).
The following statement: "Use of audio or visual recording devices, including a cell phone, is not permitted in changing areas, rest rooms or locker rooms."
The following statement: "Deck changes are prohibited."
Announcements that have "TBD" for any required information will not be
accepted.
** No application will be considered complete until the meet announcement, application, and payment (when required) are received. **
Sanction Fee: The sanction fee is $250
A check made payable to Ozark Swimming, will be required before the application is complete.

As a condition of obtaining such an approval, I and the below organization, which I represent, agree to abide and govern this event under the rules and regulations of USA Swimming and Ozark Swimming, Inc., and all other terms and conditions upon which this approval may be granted. These terms specifically include all local rules and regulations and those set forth in Article 202.4 of the current edition of USA Swimming Rules and Regulations, specific reference to Article 202.4.14, which provides that:

Additional requirements may be specified in the Ozark Policy and Procedure manual or USA Swimming rules. Please note, that the USA Swimming rules are adopted at the USA Swimming House of Delegates meeting. The requirements may be amended after the initial acceptance of a meet announcement requiring further modification.
