Thank you for your interest in the Certified Lay Ministry program. The following documents MUST be submitted along with your application. Documents will not be accepted separate from the application.

If you were formerly certified in the UMC, you will need those certificates and will have to take one GMC refresher course before applying. That can be the Refresher course or any of the five basic courses (Spiritual Formation; Basic Bible; Mission and Evangelism; Global Methodist History, Polity and Theology; or Congregational Care and Ethics). If you are new to Lay Ministry, you must take all five basic courses before applying.

Please have the following documentation ready to be uploaded while completing this application. You will not be able to leave the application and come back to complete it.

*Copies of your prior Certified Lay Servant/Minister certification(s), if applicable.

*Documentation from the staff/pastor-parish relations committee approval.

*Documentation from your local church's charge conference.

*A written recommendation by your current pastor or presiding elder to be uploaded.

*In the application below, be prepared to answer the following question as your call story. What is God doing in your life and why do you want to be a CLM?

Section IV: Authorization for Background Check
I understand that the Global Methodist Church requires a background check of all
persons seeking ordination, appointment, and/or employment. I understand that
the information that I have provided may be verified by contacting persons and
organizations with whom I had contact or who/which may have information concerning me.

I authorize the Global Methodist Church to conduct a background check consisting of,
without limitation, the following information: county, state, and/or federal criminal
records; Department of Motor Vehicle records, records of judgments and/or liens,
educational history, employment history, or other information. I also authorize, without reservation, any person, agency, or other entity contacted by or on behalf of the Global Methodist Church will furnish the described or similar information.

I waive any causes of action that I may have against the Global Methodist Church, its
members, employees, and agents, and any person, agency, or other entity providing
information in connection with the background check for libel, slander, defamation,
intentional or negligent infliction of emotional distress or for any other injury of any kind or nature caused by the gathering or supplying the described or similar information.

I authorize all such persons to treat a photocopy of this authorization as though it were anoriginal, executed authorization.
