1. I confirm that I am the legal owner or authorized by the owner of the dog(s) described in this waiver & consent form.
2. I assure Woofers Lounge Boarding that my dog(s) is in good health, is current on all required vaccinations (Distemper, Rabies, and Parvovirus), is free of fleas, ticks, lice, and has no known contagious viruses.
3. I understand that while my dog(s) is fully vaccinated that vaccines are not guaranteed and there is a small risk that my dog(s) may contract a contagious disease or illness. I agree that should this occur, I am responsible for my own dog(s) care, medical attention, and costs.
4. I release Woofers Lounge Boarding, and the owner from any and all liabilities which my dog(s) may suffer including but not limited to injury, sickness, damage, accident, or death resulting from participation in overnight boarding including outdoor walks.
5. I understand that although safety is the number one priority at Woofers Lounge Boarding, incidents of injuries may occur from playing with other dogs, which include but not limited to bites, scrapes, scratches, and sprains.
6. I understand Woofers Lounge Boarding will make every effort to contact me or my emergency contact in case of illness or injury. I authorize the owner to take my dog(s) to seek veterinary services should any illness or injuries require medical attention. I agree that I am solely responsible for any medical expenses acquired for my dog(s).
7. I represent that my dog(s) is sociable and has not harmed or shown threatening behaviours towards any person or any other dog.
8. I release Woofers Lounge Boarding from any liability should my dog(s) injure another dog or person and accept medical and legal responsibility for my dog(s) actions.
9. I agree that my dog(s) may be recorded on camera and its images may be used on Woofers Lounge Boarding social media platforms.
10. In the event that I or my authorized contact cannot pick up my dog(s) at the agreed pick-up time, I authorize Woofers Lounge Boarding to provide additional overnight services at my expense.
11. In the event that your dog(s) is not picked up on the agreed pickup date, Woofers Lounge Boarding will attempt to contact you and your emergency contact that you have provided. You will be advised that if your dog(s) is not picked up within 7 days after pickup date, your dog(s) will be deemed to be abandoned. Woofers Lounge Boarding will deliver your dog(s) to a third-party adoption partner, animal shelter, or attempt to find a new owner for your dog(s).
12. I agree to pay for any and all outstanding charges prior to or at time of pickup of my dog(s). I understand that boarding services are charged per night and will automatically be charged an addtional daycare fee for any pickups after 4pm.
13. I confirm that all information I entered in this form is true and accurate. It is understood by Woofers Lounge Boarding and Owner that all provisions of this contract shall be binding upon both parties thereunto for this visit and for all subsequent visits.
